The Complex Care Management program is a community facing program that works with our highest risk Medicaid patients to address medical, social and mental health needs. Our patient-centered care model allows us to meet patients where they are and address goals to improve overall health and well-being. Our team is made up of nurses, community wellness advocates, pharmacists and liaisons, social workers, and housing specialists. We support patients in Boston as well as many other communities throughout Massachusetts.
