OFFLINE DONATION COLLECTION FORM

Download our Donation Collection Form for collecting cash and check donations for Pet Walk. Send this form along with your donations before August 9 to:

Once we have received these donations, we will confirm them and you will see them appear on your personal fundraising webpage.  

After August 9, please bring all donations and forms with you to the registration pavillion for Pet Walk Racine at Cliffside Park starting at 8 a.m.

OFFLINE REGISTRATION FORM

For those who aren't able to register online, download our Offline Registration Form and fill one out for each person registering for the walk.  Send this form, along with your registration fee by August 9, 2023 to:

After August 9, you must register at the registration tent starting at 8 a.m. on Saturday, August 12 at Cliffside Park for an additional fee per participant type.
