OFFLINE DONATION COLLECTION FORM

Download our Donation Collection Form for collecting cash and check donations for Pet Walk.  Send this form along with your donations to:

                        Attn: Pet Walk

                        1830 Radisson St

                        Green Bay, WI 54302

Once we have received these donations, we will confirm them and you will see them appear on your personal fundraising webpage.  

 

 

OFFLINE REGISTRATION FORM

For those who aren't able to register online, download our Offline Registration Form and fill one out for each person registering for the walk. Send this form, along with your registration fee before June 2, 2023 to:

After June 2, please bring any donations with you to the registration tent starting at 8 a.m. on Sunday, June 4 at Pamperin Park.
