OFFLINE DONATION COLLECTION FORM

Download our Donation Collection Form for collecting cash and check donations for Pet Walk. Send this form along with your donations before August 22 to:

Once we have received these donations, we will confirm them and you will see them appear on your personal fundraising webpage.  

After August 22, please bring all donations and forms with you to the registration tent for Pet Walk Ozaukee at Upper Lake Park in Port Washington starting at 8 a.m.

OFFLINE REGISTRATION FORM

For those who aren't able to register online, download our Offline Registration Form and fill one out for each person registering for the walk.  Send this form, along with your registration fee by August 22, 2023 to:

After August 22, you must register at the registration tent starting at 8 a.m. on Saturday, August 26 at Upper Lake Park for an additional $5 per participant type.
