Download our Donation Collection Form for collecting cash and check donations for Pet Walk. Send this form along with your donations before July 31 to:

After August 1, please bring all donations and forms with you to the registration pavillion for Pet Walk Racine-Kenosha at Pets Park (Petrifying Springs Park) starting at 8:30 a.m.
