OFFLINE DONATION COLLECTION FORM

Download our Donation Collection Form for collecting cash and check donations for Pet Walk.  Send this form along with your donations to:

                        Wisconsin Humane Society Door County Campus

                        Attn: Pet Walk

                        3475 Park Dr.

                        Sturgeon Bay, WI 54235

Once we have received these donations, we will confirm them and you will see them appear on your personal fundraising webpage.  

 

 

OFFLINE REGISTRATION FORM

For those who aren't able to register online, download our Offline Registration Form and fill one out for each person registering for the walk. Send this form, along with your registration fee before September 6, 2024 to:

Sturgeon Bay, WI 54235

After September 6, please bring any donations with you to the registration tent starting at 9:00 a.m. on Sunday, September 8 in Sturgeon Bay.
